After you finish the carrier setup form below, we will send you a Dispatch Service Agreement along with a General Power of Attorney (Agent) via email. Once those are completed, your onboarding will be fully finalized and you’ll be ready to get started.

Please make sure to include the following documents when submitting your application:

• MC Authority 
• Completed W-9 Form 
• Insurance Certificate
